Position Summary
The Director of Network Development & Care Navigation will lead BedConnect’s efforts to expand and strengthen referral relationships across hospitals, skilled nursing facilities, senior living communities, rehabilitation centers, and post-acute care organizations.
This is a relationship-driven, growth-focused role designed for a healthcare professional with direct hospital case management experience who understands discharge planning firsthand and can effectively engage healthcare partners, build trust, and expand referral networks.
This role is ideal for an experienced hospital case management professional who understands the realities of discharge planning, patient placement barriers, and post-acute coordination firsthand. The initial focus of the role will be growing BedConnect’s network, building strategic healthcare partnerships, and serving as a trusted resource to hospital case management teams and provider partners.
As BedConnect scales, this role will evolve into broader leadership responsibilities supporting care navigation operations, patient coordination, and case management initiatives.
The ideal candidate is passionate about improving patient transitions while also being comfortable with outreach, provider engagement, business development, and strategic growth initiatives in a fast-paced startup environment.
Key Responsibilities
Strategic Growth & Hospital Partnership Development
- Build and maintain strong referral relationships with:
- Hospital case managers
- Social workers
- Discharge planners
- Physician groups
- Skilled nursing facilities
- Assisted living and memory care communities
- Home health and hospice agencies
- Expand BedConnect’s provider and referral network within targeted markets.
- Serve as a trusted liaison between hospitals and post-acute providers.
- Conduct outreach meetings, presentations, onboarding sessions, and education with healthcare stakeholders.
- Identify new partnership opportunities and strategic growth initiatives.
- Represent BedConnect at healthcare networking events, industry meetings, and community engagements.
- Track referral activity, partnership growth, and market opportunities.
Care Navigation & Case Management Support
- Utilize firsthand hospital case management experience to support complex patient transitions and placements.
- Assist patients and families in navigating senior care and post-acute placement options.
- Collaborate with discharge planners and providers to facilitate smooth transitions of care.
- Help resolve barriers related to insurance, level of care, placement availability, transportation, and timing.
- Provide operational insight into discharge planning workflows and hospital partner needs.
- Ensure compassionate, patient-centered communication throughout the placement process.
- Maintain compliance with HIPAA and healthcare privacy standards.
Leadership & Operational Development
- Help develop scalable workflows and operational best practices for BedConnect’s navigation services.
- Provide strategic feedback to leadership regarding hospital workflow challenges and market needs.
- Assist with future recruiting, onboarding, and mentorship of care navigation staff.
- Collaborate on service expansion and operational growth initiatives.
- Support continuous improvement efforts across network development and care coordination functions.
