WHAT WE NEED The Director Inpatient Care Management provides overall direction and management of Case Management. This role supports a collaborative and coordinated process with nursing, ancillary departments and physicians to ensure the effective utilization of clinical resources leading to quality, cost-effective patient care across the Healthcare continuum. The Director will supervise, and coordinate employees engaged in these activities to manage the resources and costs of high-risk or high-cost cases, including identifying appropriate cost-effective methods to manage high-risk, high-cost diagnoses and care plans.
WHAT YOU CAN EXPECT
Oversee the day-to-day operations; ensures appropriate delivery of case management, utilization review, and social work services.
Manage the clinical staff responsible for gathering clinical information to assess, expedite care needs, and post-acute transitional needs
Discuss cases with treating physicians and other healthcare professionals to understand plans of care better
Develop and manage one or multiple departmental budgets, including making budget allocations, approving expenditures and ensuring expenses are within budget
Supervise managers, supervisors, and leads within one or multiple departments and manages the performance of individuals through ongoing coaching, feedback and development to motivate, engage and drive a high performing team
Perform the CMS CoP Utilization Review and Discharge Planning
Oversee the Healthcare Financing Administration (HCFA) Quality Management Guidelines
Monitor and document KPI metrics towards goal achievement related to Initial Assessments, Initial LOC reviews and reconciliation, Initial Discharge Assessment completion, and PA review completions
Monitor, document and communicate the out-migration of all patients going to non-PAC partners.
QUALIFICATIONS
Registered Nurse license (RN) - Required
Bachelor’s degree in Nursing -- Required
Master’s -Preferred
10+ years’ experience in hospital case management experience - Required
7 years’ experience as Manager level or above - Required
