Supervisory Social Work, Military to Veteran Transition Program
The Post-9/11 Military2VA (M2VA) Case Management Program Supervisor (formerly OEF/OIF/OND and TCM) oversees all transition activities and the coordination of care and services for Post-9/11 Service Members and Veterans (SM/V's) treated at the VA Salt Lake City Health Care System (VASLCHCS). Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem solving. The M2VA Program Supervisor has programmatic and administrative responsibility for facility staff including social workers or nurse case managers and transition patient advocates who provide services to Post-9/11 SM/V's.
