Director of Case Management | RN or LCSW | Flagship Acute Care Hospital | Georgia
We are seeking a proven, high-visibility Director with a track record of turnaround and transformational leadership. This position is ideal for a leader who excels in restoring stability, elevating team performance, and partnering across clinical and operational groups to ensure patients progress safely and efficiently through their hospital stay.
This leadership opportunity is based within a well-established non-profit acute care health system located in Georgia. LOCATION: Georgia - This on-site leadership role requires permanent, full-time relocation to the community
ABOUT THIS OUTSTANDING CAREER OPPORTUNITY
- Reports to a highly engaged and supportive System Vice President
- 44 departmental FTEs to include two supporting Managers
- Provide oversight of case management, care coordination, utilization review, and transitional care services during a period of significant hospital growth
- Lead and support teams responsible for social work services, discharge planning, utilization review, care management, appeals, and management of complex patient populations
- Develop and implement strategies that support efficient patient flow, length of stay management, and continuity of care
- Collaborate closely with nursing leadership, medical staff, and interdisciplinary partners to align care delivery practices
- Ensure regulatory readiness through consistent documentation standards and compliance with state and federal requirements
- Support initiatives focused on quality outcomes, operational efficiency, and responsible resource utilization while stabilizing and rebuilding a committed Case Management team following leadership turnover
- Serve as a visible leader who promotes patient advocacy, service excellence, and a culture of accountability and collaboration
- Partner with senior leadership to advance organizational goals related to care coordination and performance improvement
