Care Management Intern
PRIMARY FUNCTION The care management intern works under the direction of a Center for Virtual Health RN providing supportive care management functions, including, but not limited to, patient education, payer authorizations, post-discharge and/or follow-up phone calls to patients to ensure timely delivery of equipment and services, completion of health questionnaires with patients, and entry of quality measure data into software systems.PRINCIPAL DUTIES AND RESPONSIBILITIES: Provides patient education to members, patients and families as directed by the primary Center for Virtual Health RN.
