Registered Nurse (RN) -- Care Coordination and Integrated Case Management (CC&ICM)
The Nurse (RN) CC&ICM will partner with the CC&ICM Social Worker to implement the national CC&ICM framework which serves a patient population with highly complex health and mental health problems to ensure they receive the right level of care coordination support based on their prominent need. The RN CC&ICM Initiative Coordinator has both clinical and administrative responsibility to ensure effectiveness of services, application of latest VHA guidelines and modification of service patterns. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
