Nurse (Mental Health Specialist Nurse Case Manager)
The Mental Health Intensive Case Management (MHICM) Mental Health Specialist Nurse Case Manager (NCM) practices within the VA Portland Health Care System (VAPORHCS) Mental Health and Neurosciences Division. The incumbent is an integral member of the MHICM interdisciplinary team managing a caseload of Veterans with severe and persistent mental illness, including schizophrenia, schizoaffective disorder, bipolar disorder, PTSD, Major Depressive Disorder, substance use disorders, other psychiatric illnesses and medical comorbidities. The incumbent provides intensive community case management to MHICM Veterans to promote optimal levels of independence, functioning and wellbeing within the community.
