Senior Social Worker - Risk Mitigation
The Risk Mitigation Social Worker is responsible for the planning, coordinating, and monitoring care for Veterans in Non-Institutional Care (NIEC) Programs, including In Patient and Out Patient Community Mental Health, Skilled Home Health Care (SHHC), Community Adult Day Health Care (ADHC), Homemaker/ Home Health Aide (H/HHA), and Respite programs within Consultative Care Service (CCS) and programs providing either Medical and/or Mental Health care to identified High Risk Veterans. The Risk Mitigation Social Worker must perform at the advanced clinical level and has a working knowledge of community program and resources.
