The VA Birmingham Health Care System (BVAHCS) is a Complexity Level 1a facility and provides extensive primary, secondary, tertiary, and quaternary care in outpatient, inpatient, and community settings. BVAHCS is a multi-site health care system that includes a major medical facility in Birmingham, Alabama, nine Community Based Outpatient Clinics. Community Care Network (CCN) Social Worker (SW) is assigned to Geriatrics and Extended Care Service as a member of the oversight team which also consists of an Occupational Therapist and a Registered Nurse.
