Intensive Case Management - Oncology Case Manager (flexible-hybrid)
DescriptionCancer patients at high-risk for unplanned hospital admissions, ED visits, and readmission. Many of these events are preventable with high initiative-driven nurse led intervention, such as early recognition treatment related side effects, symptom triage, rapid escalation to Oncology team, and coordinator discharge planning. Under the direction of the Assistant Director, the ICM Nurse Case Manager - Oncology performs complex care coordination for high-risk MA cancer patients.
