SUMMARY OF POSITION:

The Manager of Case Management is a member of the Management Team. This individual has twenty-four hour accountability for care management and social work, managing within the parameters of the annual personnel and supply budget. The Manager assures compliance with all requirements of regulatory bodies.

MINIMUM QUALIFICATIONS:

• Current PA nursing license or MSW
• Five (5) years of clinical or case management experience across multiple clinical areas 
• Three (3) years of leadership experience 
  
• Certification in Case management with in two (2) years of hire.
