The PACT Registered Nurse practices as a Care Manager for an assigned panel of Veterans in the outpatient clinic. The focus of this role is to promote and assist with optimum physical, spiritual, and mental well-being of the Veterans with an emphasis on chronic disease states. This includes providing initial and ongoing assessments, collaborating with extended PACT members and actively partnering with the Veterans to develop meaningful plans of care specific to the Veterans needs/goals. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
