Case Manager - Case Management

SUMMARY: This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services available to meet the health needs for our patients in the Charlie Mitchell Clinic. Coordinates services provided to a specified group of patients, ensuring that the care is provided in an appropriate setting, and that the severity of illness and the intensity of service is consistent with the patient's clinical condition. Collaborates with the health care team members to ensure that the care provided meets desired patient outcomes with appropriate utilization of resources within expected time-frames across the continuum of care. This position practices within the scope of the Nursing Practice Act, and in accordance with the policies on Valley Children's Hospital.

QUALIFICATIONS:

EXPERIENCE:

• Two years nursing experience required
• Previous hospital Case Management and/or Utilization Management experience is highly desirable
• Bilingual skills are preferred

EDUCATION/LICENSES/CERTIFICATIONS:

• Associate's Degree in Nursing required, Bachelor's Degree preferred
• Valid California RN license required

POSITION DETAILS: Part- time, 40 hours per pay period, weekend call required and shared on rotation

PAY RANGE: $49.00 - $74.00
