The Renal Nurse Case Manager is responsible for working in close collaboration with the Chief of Nephrology and Nephrology team to ensure and provide program services to patients requiring chronic kidney disease management. The Renal Nurse Case Manager is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
