This position as an Integrated Case Management Nurse aligned under Primary Care at the Chillicothe VA Medical Center. The incumbent is accountable for assessing, planning, and implementing patient care. The RN coordinates the episodic care of Veterans who need service(s) not able to be provided by the institution in a timely manner ,in conjunction with the primary care provider and/or the physician managing the specialty service. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
