The Specialty Nurse Case Manager works with the Specialty Services to develop and implement comprehensive patient care for the adult/geriatric patient population assessing, planning, implementing and evaluating activities and programs. The incumbent will work primarily with Veterans followed in pulmonary clinic; however may also provide coverage in other clinic areas.
