Atlantic Health System, is seeking an In-patient PD Care Manager for their Newton Medical Center.
Responsibilities
Essential Functions:
Job Function: 01. Responsible for facilitating and coordinating the care delivered to an assigned group of patients through multidisciplinary and patient/family collaboration to ensure that quality and cost effective outcomes are delivered within appropriate length of stay (LOS) parameters. Coordination involves assessment, planning support and evaluation of patient care and related outcomes. Activities to be performed include patient/family interviews, clinical resource management, utilization review, coordination of discharge planning activities and assurance of comprehensive clinical documentation for the age-specific population. Participates in interdisciplinary patient care rounds and acts as resource to team members in all regulations pertinent to LOS, payment and discharge planning. Identifies and works to overcome quality of care issues, over or under utilization of resources, barriers to the progression of care and delays in discharge. Formulates, coordinates, and implements discharge plans as appropriate and works collaboratively with nurses, physicians, social workers, and other members of the healthcare team to develop and implement an efficient plan of care that leads to a safe, timely and effective discharge.

Requirements:
Required Education and/or Experience:
Graduate of an accredited school of nursing, Active NJ RN License, BLS Certification, Note: 2014 new hire/transfer must be BSN degree. External RN hires or internal transfers into an RN position who do not possess a BSN degree, will be required to sign an agreement that they will obtain their BSN degree within five (5) years from the date of hire.
If they do not obtain their BSN degree within such 5-year period, they will be subjected to disciplinary action up to and including termination.
