Department DescriptionThe PATH program (Projects for Assistance in Transition from Homelessness) is a program designed to provide outreach, engagement services and case management to individuals who have been identified by the state as having a serious mental illness who are homeless, at risk of homelessness or unstably housed. This position locates these individuals and then wraps services around them in hopes of getting them connected to outpatient behavioral health services and other services as needed. The Forensic PATH program's overall goal is to divert individuals from the criminal justice system and instead help them get the services they need.
