RN Care Coordinator - Registered Nurse Case Management - Penn State Health St. Joseph (Reading, PA)
JOB SUMMARY:
The RN Care Coordinator coordinates the care and service of selected patient populations across the continuum. Works collaboratively with patient, family, physician and other members of the health care team to achieve the highest quality clinical outcomes with the most cost effective use of available resources. This position assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient treatment and transitional needs. Is knowledgeable about various age-related care protocols and functions within their scope of practice. Teamwork is a vital component through effective collaboration efforts.
MINIMUM QUALIFICATION(S):
Two (2) years nursing experience in acute care hospital required
Currently licensed to practice as a registered nurse by the State of employment
AHA BLS prior to end of orientation period
KNOWLEDGE, SKILLS, & ABILITIES:
Familiarity with case management role in health care setting.
Knowledge of medical terminology, utilization management criteria, community resources, and health care reimbursement systems.
Ability to analyze data, apply critical thinking process to problem-solving.
Demonstrated capacity to work with inter- disciplinary team (especially physicians) and communicate effectively. Moderate computer skills required.
PREFERRED QUALIFICATION(S):
Bachelor's degree preferred
Case management certification (CCM or ACM) preferred
Previous experience in case management, discharge planning or utilization management role preferred
Knowledge of InterQual and/or Milliman criteria is preferred
