The Transition RN Case Manager is accountable for adhering to established policies and standards of care/practice, and demonstrating leadership in delivering and improving holistic patient care through collaborative strategies with others. The Transition Team is a multidisciplinary team that addresses the healthcare needs of Veterans receiving Inpatient Care, Outpatient Primary and Specialty Care, and Extended Care Services. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
