The incumbent is an expert RN Care Manager within the Patient Aligned Care Team (PACT). Incumbent is a self-directed professional nurse who assumes a leadership role in the direct and indirect management of a group of patients through the coordination of care with physicians, nurse practitioners, medical residents/students, other health professional and hospital systems. The role resides throughout all sites within primary care nursing and reports directly to the site Nurse Manager. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
