The PACT RN Care Manager practices as a Registered Nurse under the guidance of the Nurse Manager for Primary Care within MT VA Health Care System and supports the mission and vision of the Healthcare System. Primary Care provides acute and chronic care needs for MT VA Health Care System including a wide variety of services for Veterans. The PACT RN Care Manager applies PACT principles of proactive, personalized, patient centered care and continuity of care coordination. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
