The Social Work (SW) CC&ICM Program Coordinator will partner with the CC&ICM RN to implement the national CC&ICM framework which serves a patient population with highly complex health and mental health problems to ensure they receive the right level of care coordination support based on their prominent need. The SW CC&ICM Program Coordinator has both clinical and administrative responsibility to ensure effectiveness of services. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
