The Patient Centered Medical Home (PCMH) Model is a patient-driven, team-based approach that delivers efficient, comprehensive, and continuous care through active communication and coordination of healthcare services. PACT/Primary Care Social Workers provide outpatient social work services and work closely with the primary care team.
