The Coatesville VA Medical Center (CVAMC) is currently recruiting for a full-time Certified Registered Nurse Practitioner in the Medicine and Geriatric Service. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Nurse Practitioner Specific Requirements: A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Experience: In addition to meeting the basic requirements as stated above, you must have the following: Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience; and, meet the following practice dimensions: Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning. Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making and evaluating outcomes. Functioning as an expert in clinical practice and/or in areas related to the assigned roles and responsibilities. Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within area of expertise. Guiding, developing and supporting staff from a leadership perspective. Using professional standards of care, scientific evidence and practice to evaluate programs and/or service activities. May be required to work with assaultive or combative patients. NOTE: Must have Basic Life Support (BLS) certification prior to assuming clinical duties. The BLS requirement is satisfied by successfully completing an American Heart Association (AHA) BLS Healthcare Provider Course or Military Training Network (MTN). BLS courses are normally offered within 1-5 days of entry on duty. No online certifications will be accepted. ["The position is in the Medicine and Geriatrics Service at the Coatesville VA Medical Center, PA. The duties and responsibilities are carried out throughout the medical center including all clinical and other patient care areas involved with the service, and as assigned. This position has a focus in geriatrics but will have the same CRNP coverage responsibilities of rotations throughout other clinical services. Major duties include, but are not limited to: History and physical examination. Ongoing inpatient and outpatient care for assigned patients within privileges granted. Order diagnostic studies and tests. Initiate and expedite requests for specialty consultations. Update and summarize patient's medical record, change orders when appropriate, and seek consultation with physician or specialist if clinically needed. Provide education and counseling of patients and families and preventive health care, medical treatments, and reasons for diagnostic testing and goals of care. Provide clinic coverage in other clinics if needed. Assessment, diagnosis, treatment and outcome measures of healthcare issues, histories, and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies for patient at home. Consults as applicable with other providers in primary and specialty care to assist in the care of geriatric patients as needed. Perform evidence-based practices to enhance symptom management of geriatric patients to include Palliative and end of life care when appropriate. Consults with the interdisciplinary care teams to establish a written plan of care and participating in interdisciplinary team meetings. Respond to critical lab/diagnostic test result notifications in a timely manner. Participation in staff meetings, hospital committees, and peer review process as applicable. Demonstrate effective communication both down to staff and up to leadership, to include issues that urgently affect Veteran patient care, safety, and/or satisfaction. Participate in the FPPE/OPPE process. Adheres to employee health requirements. Participate in Clinical Pertinence Reviews (routine medical record reviews). Clinical Management including indirect patient care: open encounter & open consult management, CPRS documentation rules and requirements, compliance with relevant performance measures, and participation in the development of the new focus on the Cultural Transformation, Utilization, Quality, and Patient Safety oversight, process improvement (system redesign).
