The PACT (Patient Aligned Care Team) Care Manager uses the nursing process and evidence-based practice to collaborate with the Core Team and Expanded Team in providing care to the patient. The PACT Care Manager Registered Nurse falls under the clinical pathway, as a direct care nurse, and reports to the Clinical Manager, Ambulatory Care Service Line. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
