The Lead HUD-VASH Case Manager, a professional social worker, functions as Eastern Kansas Health Care System's (EKHCS) Lead for Housing and Urban Development-VA Supportive Housing (HUD-VASH) Program. The HUD-VASH program which is located at both the Topeka and Leavenworth campuses is a collaborative effort, supported through HUD Section 8 rental assistance vouchers and VA' s provision of intensive case management services. The closing date of this announcement has been extended to 5/13/2022. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Education: Successful completion of master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. (Transcripts must be provided). Licensure: Must be licensed or certified by a state to independently practice social work at the master's degree level. (License must be provided). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Experience working with homelessness and with Veterans is preferred. Duties include but not limited to: Responsible for data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements for the HUD-VASH program. Develops working relationships and agreements with other organizations and directly coordinates with the designated PHA regarding housing voucher application and award process for each veteran. Development of program procedures. Oversee the determination of when veterans are eligible for HUD-VASH services as well as when they are no longer in need of case management and notify public housing authority. Ensures that the organization's strategic plan, mission, vision and values are communicated to the HUD-VASH team and integrated into the team's strategies, goals, objectives, work plans and work products and services. Communicates to the HUD-VASH team the assignment, project, problem to be solved, actionable events, milestones, and/or program issues under review, and deadlines and time frames for completion. Coaches the HUD-VASH team in the selection and application of appropriate problem solving methods and techniques, provides advice on work methods, practices and procedures, and assists the team and/or individual members in identifying the parameters of a viable solution. Identifies, distributes and balances workload and tasks among employees, making adjustments as necessary. Monitors and reports on the status and progress of work, checking on work in progress and reviews completed work to see that the supervisor's instructions on work priorities, methods, deadlines and quality have been met. Maintains program and administrative reference materials, project files and relevant background documents and makes available policies, procedures and written instructions from the supervisor. Prepares reports and maintains records of work accomplishments and administrative information, as required, and coordinates the preparation, presentation and communication of work-related information to the supervisor. Represents the HUD-VASH team in dealings with the supervisor or manager for the purpose of obtaining resources, and secures needed information or decisions from the supervisor on major work problems and issues that arise. Estimates and reports to the HUD-VASH team on progress in meeting established milestones and deadlines for completion of assignments, projects and tasks, and ensure that all team members are aware of and participate in planning for achievement of team goals and objectives. Leads the HUD-VASH team in assessing its strengths and weaknesses and provide leadership to the HUD-VASH team in exploring alternatives and determines what improvements can be made. Resolves simple, informal complaints of employees and refers others, such as formal grievances and appeals, to the supervisor or an appropriate management official. Advises supervisor about performance management issues/problems and to recommend/request related actions, such as: assignments, reassignments, promotions, tour of duty changes, and peer reviews. Establishes, implements, and maintains referral and screening procedures for potential HUD-VASH veteran participants. Conducts and participants in outreach activities including field interviews, assessments and referrals. Provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. Assigns cases to the HUD-VASH staff and oversees the development of psychosocial treatment plans, actively involving the veteran and their family or significant others. Oversees the provision of direct mental health and substance abuse services for veterans and family members or significant others in support of the veteran's treatment. Receives and completes consults requesting services from interdisciplinary team members and from other mental health professional on complex, difficult cases. Documents in the medical records utilizing paper and/or electronic formats, including psychosocial assessments, diagnostics, treatment, progress notes, follow up and referral. Provides training to students, residents, interns and fellows from other disciplines and conducts formal workshops and classes for staff. Other duties as assigned.
