This full-time opportunity will allow you to play an integral role in coordinating and assessing the comprehensive care plans for our patients. Here, you will be able to:

• Collaborate with a cohesive and diverse team that encompasses all facets of patient care. This includes, but isn’t limited to, physicians, PT/OT staff, pharmacists, respiratory, dietary, nursing and social workers.
• Provide the necessary oversight and management of our patient’s progression of care throughout hospitalization.
• Work with our care teams and patients to determine the appropriate resources and services that should be utilized to ensure safe, effective and efficient transition plans when patients are discharged from the hospital.
• Be part of a Magnet®-designated nursing team, work with world-class physicians, contribute to system governance and improvement initiatives, and participate in medical training or research.

We’re seeking candidates with a current Florida RN license, preferably with a BSN degree, and two years of recent clinical RN experience within a hospital-based setting. Case management experience and professional board certification within a specialty area are preferred. Candidates who do not have a BSN or specialty certification in their area of care will be required to obtain them within two to three years after hire. Successful applicants must pass an FBI Level 2 screening to be eligible for this role.
