GENERAL SUMMARY: Under general supervision of the Chief Worker, provides patient assessment, functions as a member of a multidisciplinary team, and provides discharge planning. Provides services to patients of all ages. PRINCIPLE DUTIES AND RESPONSIBILITIES: 1. Provides patient assessment.
