The PACT RN Care Manager is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. They will assume responsibility for the coordination of care for the patient in the Primary Care Department and assists with the coordination of care with specialty services, surgical services, other ancillary services, and VA Care in the Community as needed. To qualify for this position, applicants must meet all requirements by the date of hire.
