Home Based Primary Care (HBPC) is a home care service that provides longitudinal care for rural and highly rural, complex, and homebound Veterans. The incumbent is responsible for providing nursing care, care coordination, and case management on a scheduled and as needed bases for Veterans living in the northern PA area. Types of care provided in the home include but are not limited to wound care, catheter care, lab draws, medication management and oversight, education, and end of life care.
