Advocare is seeking a Director of Care Management to lead our team of professional Care Managers!
Join a well established leader in the Private Senior Care Management arena. The ideal candidate must be a RN in Florida with ample knowledge of medical resources in the local community. This is a Full-time exempt, role. You must be comfortable interfacing with Clients and Team Members on a daily basis. The office is located in Delray Beach, FL
In this role, you will be expected to:
Manage activities of Care Management Practice office and field operations from our Delray Beach main office location
Manage Intake process and oversee all new client communication and inquiry status
Review all completed new client packets and assign appropriate medical care manager
Discuss incoming cases with care managers and assist care managers in setting up care plan
Review all reports and sign off on any new documentation and assessments
Be available and responsive to troubleshoot with field care managers to avoid issues that might arise
Coach office staff on how to handle client complaints or concerns. Direct proper management personnel to handle the specific concern directly or delegate appropriately
Handle specific clinical care management issues with clients/family to resolve and diffuse any concerns
Direct care managers to appropriate referral sources
Review of clinical team’s case notes, medication lists, updated care plan reports to ensure quality of services delivered
Manage, train and review field care mgmt. staff and engage in ongoing peer review on an as needed basis. Organize and assist in planning of new business relationships
Work in tandem with other team members to plan timing and case collaboration
Communicate with field staff via email, in person and plan agenda and implement all staff meetings
Engage and meet with upper management to assist in achieving overall goals of organization
Assist in all company wide projects and plan responsibilities accordingly
Participate in an ongoing call rotation after hours to receive intake calls from potential clients and assist field care managers who might need advice and assistance managing a particular and urgent situation.
Attend community events when invited to represent organization

Skills/Qualifications and Requirements:
Bachelors of Science in Nursing and/or Masters of Science in Nursing and/or other related field
5+ years minimum of a Management or Leadership role in clinical setting
Formal Case Management Experience in a Multidisciplinary clinical setting ie hospital, insurance company, case management organization preferred
Case Management Certification preferred (CMC) or (CCM) but not required
Strong written and verbal communication skills
Ability to educate, train and teach care management principles as it relates to practice
Customer service and leadership based resolution experience a must
Ability to engage and diffuse situations with clients and team members
Ability to prioritize, plan, flex and manage very busy work environment
Team building and conflict resolution skills required
