The Registered Nurse Care Manager (RNCM)/ Triage nurse functions in the Primary Care Clinic which utilizes a Patient Aligned Care Team (PACT) model. The PACT RNCM/Triage nurse will be responsible but not limited to triaging unscheduled walk in patients for the clinic and work as the primary PACT RNCM on a team. The RNCM/Triage nurse is responsible for organizing and directing team members to achieve desirable patient outcomes through day-to-day and/ or telephonic case management.
