As a full-time Licensed Social Worker in our Care Management Department, you will support the vision of the Total Healing Environment while providing the timely and accurate identification of patients’ psychosocial needs. You will also develop and implement plans of care for post-hospitalization.
Master’s degree in Social Work (MSW) from an institution accredited by the Council on Social Work Education or an equivalent foreign degree as determined by the Foreign Equivalency Determination Service of the Council on Social Work Education
Current Arizona Licensed Clinical Social Work (LCSW) or Licensed Master Social Worker (LMSW) by the Arizona Board of Behavioral Health Examiners
Excellent interpersonal and communication skills
Basic computer literacy and proficiency in Microsoft Windows
Basic proficiency with MS Office (Outlook, Word, Excel)
1 year of experience as a social worker in a hospital, psychiatric or other medical environment
Electronic Medical Record (EMR) experience preferred; Cerner strongly preferred
