This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The Weight Loss Clinic Telehub Specialty RN Care Manager provides coordination of care and management of patients in the areas of the inpatient surgery ward, the outpatient surgery ward, outpatient clinics and the virtual and telephone clinics. The incumbent is clinically and administratively responsible to the Nurse Manager or the Chief Nurse of Ambulatory Care Services Work Schedule: Monday thru Friday 8:00am to 4:30pm; to meet the needs of the patient and program needs Telework: (This is not at virtual position for the assigned duty/report to location is at the Palo Alto Campus. Financial Disclosure Report: Not required The major duties and responsibilities include, but are not limited to: CLINICAL: Provide routine patient care and management in the clinic setting. Triage patients with acute and chronic weight loss complaints, in consultation with Weight Loss Clinic staff and according to standardized procedures. Request and follow-up on diagnostic studies as indicated per request of Weight Loss Clinic providers. Serve as case manager for Weight Loss Clinic patients with complex needs; to coordinate and facilitate their Weight Loss care (e.g. Patients referred to Weight Loss Clinic from within and outside of VAPAHCS, interfacility patients, integrating care with other medical specialties). Triage new consults, coordinate clinics to ensure visits are scheduled into appropriate clinics with appropriate studies requested by Weight Loss Clinic providers. Coordinate patient lodging, meals, and transportation needs for appointments, treatments, and surgeries. Participate in inpatient surgical care as needed. Serve as a resource person for patients and families for peri-operative and post-discharge care. Provide patient and family education. Serve as a clinical resource person for other nurses and other services regarding clinical issues concerning Weight Loss Clinic patients. Serve as a contact/resource person within the VAPAHCS and other VA facilities for the Weight Loss Clinic program to facilitate further testing, procedures, and follow-up as ordered. Notify supervising providers regarding patients with atypical findings or problems that are not resolving as expected. Serve as the Weight Loss Clinic Navigator in providing assistance and guidance to the patient and family members through weight loss/bariatric treatment and facilitate ongoing care that is required. Serve as the Weight Loss Clinic Telehealth/Televideo coordinator to assist providers in facilitating consults, follow-up appointments and any ongoing needs around Telehealth. Other duties as assigned. Duties continued in Requirements section ADMINISTRATIVE & ADDITIONAL DUTIES: Serve as a primary Weight Loss Clinic patient advocate. Provide service recovery to address patient and family concerns. Work in conjunction with the General Surgery schedulers to coordinate weekly operating room schedule for bariatric surgery to include contacting patients, arranging for pre-operative workups, surgery scheduling, arranging for appropriate peri-operative care and arranging for lodging reservations as needed. Respond to telephone calls and secure messaging for Weight Loss Clinic. Provide timely triage and review by appropriate provider. Contact patients who missed clinic appointments and reschedule appropriately to maintain continuity of care. Assist in completion of short-term disability, Family Medical Leave Act and Compensation and Pension paperwork at patient request. Assist with submission of patient lists for multidisciplinary clinical conferences (e.g. radiology rounds, tumor board). Assist in the administrative closure of clinic encounters in electronic health record (CPRS). Maintain a bariatric surgical patient database. Coordinate and follow-up on community care referrals and facilitate ongoing needs following referrals as appropriate and per VA guidelines. Provide service recovery for patients who may want to repatriate to the VAPAHCS. Apply current research by contributing to Health Care System policies, procedures and protocols. Maintain clinical competency by attending relevant continuing education programs and involvement in professional organizations. Coordinate care related to COVID (e.g. perform COVID screens, order COVID testing as required/ordered). Assist with scheduling telehealth, telephone and video visits and follow-up Participate and manage program development as related to innovation within the Weight Loss Clinic department. Participate in audits and lead quality improvement projects within the Weight Loss Clinic department. All other duties assigned by Nurse Manager or Service Chief as deemed appropriate.
