The Admissions Nurse reports directly to the WHEN Supervisor Nurse Manager at the Northport VA Medical Center and works closely with the BMS Coordinator to efficiently manage patient flow into and throughout the facility. Duties/ Responsibilities include, but are not limited to: Performs clinical nursing assessments, then documents findings accurately utilizing the applicable programs. Communicates patients' physical and psychosocial needs, and initiates nursing care plans as appropriate. Responsible for providing back-up coverage for the Bed Coordinator. When serving in the Bed Coordinator role, the incumbent is responsible for utilizing, maintaining, and verifying transactions on the Electronic Bed Management System to manage internal and external bed requests and transfer requests. Possess excellent computer program skills to include: Microsoft word, excel and access, as well as email. Will act as the Bed Czar for the facility. Maintain an electronic log of admission/transfer requests during normal business hours. Responsible for updating individual patient/resident data to reflect their care needs. Responsible for critical thinking skills in assessing patient and clinical information and applying InterQual Criteria to admission requests, to place patients in the appropriate level of care. Responsible for utilizing data resources to generating the following lists/reports: Scheduled Admission List, Transfer-Out List, Report of Contact Log and other reports as assigned. Responsible for educating internal and external customers on the admission/transfer process. Responsible for communicating pertinent data to internal and external customers in a timely, professional, courteous, and effective manner. Work with the Emergency Manager to account and coordinate movement of patients in the case of emergency. Exemplary work ethic is required for this position. Work with the providers/utilization staff to effectively admit patients/residents to appropriate level of care. This RN is also responsible and accountable for maintaining training required and may be assigned additional duties as area needs require. Work Schedule: The primary tour is 11:30am to 8pm, but will vary based upon coverage needs (e.g. 8a-4:30p, 8:30a-5p, etc.). Financial Disclosure Report: Not required
