This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The incumbent is the Coordinator of the Contract Adult Day Health Care (CADHC) Program, Homemaker/Home Health Aide & Home Respite Programs for the VAPAHCS. The scope and impact of this position are far reaching and thus require an extremely knowledgeable, diplomatic leader who has strong administrative capabilities, takes initiative with respect to program planning, resource allocation and who has exemplary communication skills. In addition, the incumbent represents the VAPAHCS regarding both clinical and administrative extended care issues in the community with contract agencies. The incumbent provides direct patient care services as well as coordinating the daily operation of the program, developing policies and procedures for program operation and preparing reports and statistics for facility, VISN, and national use. Provide clinical social work services to veterans in the Contract Adult Day Health Care Program, Homemaker/Home Health Aid and the Home Respite Program and other non-institutional VA programs with high quality case management, clinical treatment, coordination of services and advocacy. The incumbent will coordinate linkage with appropriate VA and community service providers / agencies as needed by the client. This is accomplished in collaboration with other members of the interdisciplinary treatment team as appropriate.
