1. Maintains established departmental policies and procedures, objectives, quality improvement program, safety, environmental and infection control standards. 2. Provides input for the evaluation of work performance and makes recommendation for personnel action. 3. Assumes duties of the Department Director in her/his absence. 4. Assumes duties of the Hospice/Home Health Patient Care Coordinator in her/his absence. 5. Manages care of patient caseload. 6. Assigns patients to skilled nursing and home health aide staff, ensuring continuity of care. 7. Evaluates and assesses the patient and his/her home environment. 8. Communicates with the physician and other community health related persons in order to obtain physicianâ��s orders and coordinates the care plan, and reports any changes in the patientâ��s condition. 9. Assists in developing and implementing the patientâ��s care plan. 10. Monitors and ensures accuracy of recording on patient medical records. Maintains all required reports, records, statistics, etc. 11. Adjusts staffing to meet needs and to provide adequate coverage of caseload. 12. Screens incoming referrals and determines if appropriate for service. Develops and delegates referral follow- ups. Refers inappropriate cases to agencies capable of meeting patient needs. 13. Coordinates patient care among RNs, home health aides, therapists and social worker. 14. Enhances professional growth and development through participation in educational programs, current literature, inservice meetings and workshops. 15. Directs and supervises Home Health Aides, RNs, Therapists and LVNs. 16. Provides home evaluation findings and physicianâ��s orders to various insurance companies when prior authorization is necessary. Assists the insurance companies in case management activities. 17. Demonstrates sensitivity and competence in the care of the ill and/or dying adolescent, adult, geriatric and pediatric patients relative to determining each patientâ��s age related physical, psychological and cultural needs.
Diploma in nursing/social work
1 year of healthcare experience in an institutional setting
Prolonged, extensive or considerable standing walking
Use of a personal computer and C.E. device
Current CA RN license and/or license social worker
PT, RN, LCSW preferred
Current CPR certification
Valid CA driverâ��s license
