Your role: In this role you will work in a centralized care center in the Green Hills Office Building providing patient care telephonically. You will enjoy the opportunity to be part of a team of RNs, social workers, case managers, and pharmacists who as a team ensure a successful care transition for our post-hospitalized adult patients. The services you will provide will be education on discharge instructions, on medical issues, medications, and services needed post-discharge. You will enjoy using your critical-thinking skills through telephonic communication to avoid gaps in care and preventing an unnecessary trip to the Emergency Department or a readmission to the hospital.
Department Summary: The Discharge Care Center is a multi-disciplinary team dedicated to supporting patients through transition in care. Our department is a telephonic care center using multi-channel communication techniques to reach our patients for 30 days post-discharge. Our goal is to ensure the patients receive the services they need to successfully transition from the hospital and avoiding a readmission. Our team of nurses works at the top of their licensure using high level critical thinking skills to problem solve for our patients. Our patients know we are here for them 24/7 and are very grateful for this service.
Work hours: PRN Days or Nights As a PRN Registered Nurse, you will work a 12-hour shift. You are eligible for weekend and night shift differential pay.
