Functions as part of an interdisciplinary team to maintain continuity of patient care for identified patient populations from admission to discharge, utilizes appropriate criteria for inpatient care and plans timely discharges. Responsible for coordinating patient's clinical plan of care guided by Patients/Families, Physician Providers, and other members of the inpatient team while adhering to guidelines and contracts from regulatory bodies and insurers. Plays a vital part in throughput.
Bachelor's degree in Nursing with 3 years of related experience required.
5 years of direct care nursing experience required.
Active license to practice as an RN in the Commonwealth of Pennsylvania required.
KNOWLEDGE, SKILLS, & ABILITIES:
Ability to establish and maintain a collaborative relationship with internal and external health professionals.
Excellent written and oral communication skills.
Ability to use problem solving techniques to achieve desired results and effective change.
Ability to function effectively in a dynamic and rapidly changing environment.
Ability to organize, prioritize, and time manage responsibilities.
Basic computer skills with effective keyboarding abilities.
Previous experience in case management, care coordination, outcomes management or discharge planning preferred.
Certification in case management from a nationally recognized organization preferred.
