Adolescents attend this program struggling with mental health issues. This is a short-term Day Program. A patient's average length of stay is 4 weeks. This is a dynamic interdisciplinary team of providers.
Provides assessment, education and theraputic interventions related to health/wellness issues for clients and their famlies. Serves as a liaison between providers and the interdisciplianry team. Communicates effecetively verbally and in writing with the interdisciplinary team and external systems. Participates in the development and implementation of the treatment and discharge plan. Functions as an interdisciplanry team member. Responsible, as assigned, for the case management of a caseload. Facilitates groups, provides individaul and group psycho education.
Current MN RN Licensure and BLS required Having both adolescent and behavioral experience preferred.
