The Greater Los Angeles VA Healthcare Network is recruiting for two (2) Senior Social Workers in the Social Work Service (West Los Angeles Medical Center and Downtown Los Angeles Ambulatory Care Center), Mental Health Intensive Case Management Program (MHICM). The goal of MHICM is to assist chronically mentally ill Veterans in remaining out of the hospital by providing a behaviorally oriented program of psychosocial rehabilitation, intensive case management, and community-based treatment. The major duties and responsibilities include but is not limited to: Providing advanced-level intensive case management services to assigned Veterans consistent with the goals and methods of MHICM and the VA Mental Health Initiative and with all relevant Medical Center policies and procedures. Assisting with the development of policies, reports, and statistical analysis for facility, Veterans Integrated Service Network (VISN), or national use. Conducting advanced-level psychosocial assessments and psychosocial treatment to Veterans diagnosed with severe, persistent, and complex mental illness. Assessing the psychosocial functioning and needs of patients and their family members who are experiencing a variety of psychiatric, medical, and social problems. Identifying high-risk patients and completes mandated suspected abuse reporting for dependent adults or elders. Assessing the need for conservatorship, VA Fiduciary or Social Security Payee Services, and determines the need for consultation with VA Regional Counsel regarding care. Ensuring a provision of continuity of care services within the framework established by MHICM, such as collaborating and consulting with other interdisciplinary treatment teams and programs to promote and enhance the care of patients. Performing outreach and assertive provision of services to treatment resistant patients. Ensuring mobilization of environmental and organizational supports, including family members, professional, and other interested parties, which involves undertaking individual, family and group therapies, as well as educational programs for veterans, families and staff. Participating in team staffing regarding patient selection, determination of treatment plans and goals, progress evaluation, discharge planning, and after care. Meeting with VA, community, and professional organizations and individuals as needed to carry out assigned\ developmental and implementation responsibilities. Supervising and participating in the training of Social Workers, volunteers, and students. Work Schedule: Monday thru Friday, 8:00am to 4:30pm Financial Disclosure Report: Not required
