The incumbent is assigned to Grant and Per Diem Program as the liaison within the Mental Health Service working with Veterans who are homeless and needs immediately housing. The Veteran may have substance abuse and mental Illness. He/She is a member of an interdisciplinary team that provides some case management for Veterans who present as homeless and in need of a stable housing environment. Major duties and responsibilities include: Serves as the HCHV Coordinator for the Medical Center and works under the supervision of the Homeless Program Supervisor in Mental Health Service. Serves as the Lead for the Homeless Program and a point of contact for the VISN Homeless Coordinator for all key components of the homeless program. Provides case management at homeless community-based GPD sites as described in the GPD Program rules, regulations, and directives. Provides services and oversight of the GPD funded programs. Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless veterans as well as provides current information on services and benefits available in the VA for Homeless GPD Program participants.
