The Social Work Clinical Assessor requires advanced knowledge and understanding of the services and benefits available for Veterans and caregivers from federal, state and private sector resources. As a member of the interdisciplinary Program of Comprehensive Assistance to Family Caregiver (PCAFC) assessment team, the Clinical Assessor participates in face-to-face or virtual contact with the caregiver and Veteran to assess function and the need for personal care services. FUNCTIONS OR SCOPE OF ASSIGNED DUTIES The Caregiver Support Social Worker's primary function is to complete in-home and virtual/telephone assessments as required by the Program of Comprehensive Assistance for Family Caregivers (PCAFC). Other clinical duties may include, but are not limited to: Supportive individual counseling to Caregivers; Caregiver/Veteran couple's counseling when clinically indicated-Caregiver support groups; these may occur at the facility and/or in the community a Coordination of Care and brief Case Management to assist Veterans in accessing services in a timely manner; this may include direct referrals to VA medical and mental health treatment services and to other VA and Community services (i.e., Home-Based Primary Care, Respite Program, etc.) including VA benefits, as well as to community-based services (i.e., Hospice) and other non-VA entitlement programs. Clinical triage and crisis intervention when clinically required; this includes completing all aspects of VA required risk assessments and safety planning Caregiver education workshops and training to Caregivers, VA staff and community partners regarding Caregiver issues Community Outreach at both community (Veteran Service Organizations) and VA-sponsored events (i.e., Mental Health Summit) regarding VA caregiver benefits, programs and services. Quality-improvement activities and evaluation/consultation with Social Work Supervisor and Caregiver Support Team. The Social Worker practices at the advanced level and expands clinical knowledge in the profession, supporting a facility-wide Caregiver Support Program, provides consultation and guidance to colleagues, role model effective social work practice skills, teaches or provides orientation to less experienced social workers, develops innovations in practice interventions, and provides clinical supervision for unlicensed Social Workers seeking licensure or certification.
