The Patient Aligned Care Team (PACT) Social Worker will be located in the Primary Care Clinic area providing medical Social Work services to the Veteran population enrolled at the Viera VA Outpatient Clinic. The PACT Social Worker will develop an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others. Responsible for developing a treatment plan and setting achievable treatment goals with the Veteran/family. Provides psychosocial support to a special Veteran population with highly complex health and/or mental health problems and the provision of short-term, solution focused counseling with Veterans and/or their families. Demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions. Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, Measurement Base Care, individual, family and group counseling, discharge planning, case management and other services as appropriate. Experienced in making rapid assessments and developing crisis management plans for Veterans within the VA and community. Coordinates treatment and communicates with other social workers and community program coordinators to include NIC, CNH, and TCM, Behavioral Health Programs, Homeless Program, Suicide Prevention etc., to facilitate continuity of care for Veterans. Fulfills ethical responsibilities to Veterans, colleagues and VA in accordance with relevant discipline Code of Ethics, VHA Code of Ethics, and local Ethics policies, which includes but is not limited to compliance with all laws and regulations related to confidentiality and release of information, and protecting Veterans' personal identifying information. Supervises the practice of social work students, or a less experienced social workers. Collaborates in the performance improvement processes and complies with performance measures as required by the VA for their specific population and/or program. Evaluates his/her practice on an on-going basis through participation in professional peer review, case conferences, research studies, or other organized means.
