Under the supervision of the Clinical Director of Telehealth, the Telehealth Clinician is responsible for providing clinical services and complex care management to patients through teleconferencing modalities. The Provider works to support a smooth interface between medical providers and specialty mental health services. They will also be involved in the patient care team, providing appropriate behavioral health screenings and assessments, brief psycho-social interventions, and coordinating care with the PCP and other providers.
Conducts initial screenings and assessments around mental health disorders for triage and support into the healthcare system.
Performs individual diagnostic, psychosocial, and crisis assessments, assists with individualized treatment plan design, documentation and implementation of treatment plan activity, and evaluation of outcomes.
Provides clinical knowledge of emotional disorders, chronic health, medication issues, substance use disorders, communicable diseases, advanced directives, and community referrals.
Conducts group and individual psycho-educational classes and brief, short-term therapy strategies that will help strengthen patients’ overall physical and mental health.
Actively participates in clinical presentations and/or interdisciplinary team meetings and communicates client progress and barriers with Manager.
Assists in identification of risk factors leading to increased utilization and works to reduce preventable hospitalizations, readmissions, and inappropriate utilization.
Utilizes Electronic Health Record and other tools to ensure appropriate and complete documentation and data entry of all interactions with clients, providers and other stakeholders in a timely manner.
Participates in health promotion activities for clients who do not meet criteria for intensive care management.
Establishes effective collaboration, communication and coordination among all responsible parties of an individual client’s interdisciplinary health care team.
Uses telehealth technology as necessary to provide and coordinate care at a distance.
Participates in the development of clinical telehealth workflows, policies, and protocols.

Education: Master’s degree and clinical license in a health-related field required.
Experience: Two years’ experience providing therapeutic interventions in a behavioral health and/or physical health setting. Experience working with Medicaid populations and related social/economic issues preferred.
Knowledge, Skills, and Abilities: Knowledge of integrated care, managed care, and case management required. Demonstrates support for the company’s mission, vision and values. Position requires excellent written and verbal communication skills, strong organizational and time management skills, strong interpersonal skills and the ability to handle multiple priorities. Must possess a high level of professionalism related to providing therapeutic interventions, high level care and treatment planning, and care coordination from a systems perspective. Must be able to work within a team and independently. Position requires flexibility to use complex processes and procedures to facilitate quality outcomes and/or resolutions while working with other team members with varied skills and educational background. Ability to travel to and work independently in clinic sites when needed. Must be proficient in use of electronic health record systems, video-conferencing systems, and electronic communications technologies. May be required to manage multiple priorities and projects with tight deadlines. Bilingual skills highly desired.
Licenses/Certifications: Must possess Active Colorado LMFT, LPC or LCSW license, in good standing. A valid driver's license and proof of current auto insurance will be required for any position requiring driving.
