This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The Medical Foster Home Program Coordinator manages the daily operation of the Medical Foster Home program, and provides direct patient care services in the program area. Social Work responsibilities include developing an assessment in conjunction with the interdisciplinary treatment team, the Veteran, family members and significant others, whenever possible. The goal of the assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status and safely maintain the veteran in the home. The social worker will coordinate with VA staff and community agencies to assist in problem solving as needed. Case management with members of the HBPC/SCl/D interdisciplinary team is provided in order to coordinate a collaborative effort to meet the agreed upon goals for a Veteran's treatment needs. The incumbent will serve as a liaison with the treatment team and will educate and encourage the Veteran to advocate on their own behalf, thus fostering a sense of independence and empowerment. In conjunction with HBPC/SCl/D Social Worker, incumbent will support established care plan with interventions to increase access to care/services or refer to the proper interdisciplinary team member for identified health concerns. Incumbent may offer information and referral to community resources including transportation assistance and advanced directives, economic assistance, including assistance with medical bills, financial aid services and strategies to increase income (employment, vocational rehabilitation, VA benefits, disability) and/or reduce expenses. The Social Worker is experienced in making rapid assessments and developing crisis management plans to maintain Veteran in the home and has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis. In the event of a medical or psychiatric emergency, staff will follow the protocol of the Medical Foster Home Program. Responsibilities include managing the daily operations of the program, developing policies and procedures for program operations and preparing reports/statistics for the facility, VISN21, and national use. The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation. In conjunction with the Service Chief, plans, organizes and directs administrative, operational, patient and personnel activities for the service.
