The El Paso VA is recruiting for a Health Care for Homeless Veterans (HCHV) Grant & Per Diem (GPD) Liaison Social Worker. The GPD Program, an essential and critical part of VHA, is vital for providing safe transitional housing and supportive services for homeless Veterans. This vacancy will remain open until filled. The first cut-off date is 6/08/2021. The HCHV GPD Liaison provides services to, and oversight of the GPD-funded community-based programs as outlined in the Grant Per Diem VHA Handbok 1162.01 and GPD regulations. This includes oversight of the GPD Program, assemble team of experts for inspection of the program site(s), and oversight of participants' care and case management, as necessary. The GPD Liaison participates in the development of referral processes for admission and manages waiver and extension requests for participants. Initiates/monitors the billing process for the per diem payments by ensuring that invoices are accurate and processed within policy timelines. Ensures that the clinical documentation, program required documentation, and program records are in compliance with national programs, VISN and local VA facility policy and procedures in accordance with GPD provider original grant documenting the services to be provided. Verifies the Veteran status and eligibility of program participants; verifies admission and discharge dates of program participants for billing purposes; collects and submits GPD-funded program participant data as outlined by Homeless Operations Management and Evaluation System (HOMES) procedures. Prepares clinical documentation for Veterans. Reviews and approves/denies waiver requests for Veterans that have had three or more episodes of care based on clinical assessment of the individual case and forwards a memorandum for record.
