Additional applications will be referred as needed. Provides training and information to other federal agencies, and community agencies regarding VA services. Develops and maintains working relationships with a network of service provision agencies and individuals in all areas relevant to health care for homeless needs and services. Enhanced training, insight and experience is utilized to interpret data and identify viable treatment options. Assesses high risk factors, acuity, and the need for services. Able to serve Veterans who tend to have frequent and severe crises, lack family or an adequate community support network, be poor at self-monitoring, and frequently fail to comply with instructions and treatment, or have significant deficits in coping skills and require continuing professional psychosocial support. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Incumbent maintains knowledge of Suicide Prevention policy and procedure, recognizes warning signs for suicide ideation or recent self-directed violence, and utilizes the identified screening tools to assess suicide risk. Assists transitioning Veterans and their families in understanding their rights in addition to their responsibilities. Maintains a liaison with Veteran service organizations, community groups, and others whose interest is in helping and protecting Veterans, their families, and their representatives. Prepares and maintains client records/administrative records and reports in accordance with program guidelines and policies; implements legal and ethical standard of conduct, privacy, and confidentiality. Participates in administrative, staff, clinical, and in-service meetings at the facility and at other locations as directed by leadership. Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. Works with health care system multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to homeless Veterans. Evaluates their own practice on an ongoing basis through participation in professional peer review, case conferences, research studies, or other organized means. Accepts other assignment and/or collateral duties as directed by the HCHV Program Manager/coordinator or Chief of Social Work Service to support the Mission of the WTVAHCS. The Social Worker participates in "on call" coverage for Social Work Service. Performs other related duties as assigned.
