CHA will add 42 new child, adolescent and specialized autism spectrum/neurodevelopmental beds at its Somerville Campus, bringing CHA's capacity to 69 youth beds. The new CHA Center of Excellence for Child and Adolescent Inpatient Mental Health Care at Somerville will provide a transformative continuum of patient- and family-centered care for diverse youth with mental health needs. The Center will also provide unique opportunities for training and research at CHA to identify when, where and how to positively impact trajectories of the Commonwealth's most vulnerable youth.
In collaboration with other members of the healthcare team, the Case Manager is responsible and accountable for patient trajectory by assisting the healthcare team in pacing care toward improved patient outcomes. The Case Manager role is structured around three major functions: Care Coordination, Utilization Management, and Discharge Planning. Each of these functions supports the Care Management department's goals:
Improved patient satisfaction and patient outcomes
Decreased length of stay
Decreased unit cost
Graduate of an NLN-accredited School of Nursing, BSN is required for all newly hired RN candidates or within one year from date of hire.
Current RN license in the Commonwealth of Massachusetts
Case Management certification preferred.
American Heart Association BLS.
Minimum of 5 years of broad clinical experience, predominately in psychiatric setting(s). Hospital experience strongly preferred. Case Management experience strongly preferred.
